Out of Town Form

Now that you've got your measurements:

• PRINT the form
• Please be sure to include all of your contact information
• Send the signed form along with your payment to Wishes and Whims Bridal (if using a credit card, please be sure to include your expiration date and 3 digit security code, located on the back - OR call us with that information

A few things to remember:

• A 75% deposit is required to place your order.
• ALTERATIONS will be probably be required.  These are done at YOUR expense.  
• Dresses ARE NOT custom cut to your measurements.  We take measurements to get you close to the correct size.  It does not mean the dress will fit perfectly when it arrives.
• ALL Sales are Final. 
• Deposits are Non-Refundable and Non-Transferable. 

You will receive a copy/receipt of your order, once we have received deposits from all of the bridesmaids in the wedding party.

When the dresses are received from the manufacturer, we will call the bride, and she will be responsible for notifying you that your dress has arrived.
